Linkage to Investigation Documents

There was a thread started yesterday with a link to a pdf file that had BOP documentation and test results.
The link above is for the investigation into what exactly happened, and gives multiple documents (TO IADC report from the rig, BOP testing procedures, BOP tests, etc.) that weren't availble before the hearing.
Some of it is greek to me because I don't deal with it, and some of it I know well because I deal with it every day. Makes for some interesting insight though.

the theory that the casing hanger was unseated due to building reservoir pressure behind it is advanced by BP. based on what i can figure out, it seems quite reasonable.

I've been reading through the transcripts, and my background is more in the chemical industry. Does anyone know how changes in the field are managed offshore? In the chemical industry, we use Management of Change procedures. I was just wondering if the offshore industry has procedures like this in place?

I believe someone mentioned that OSHA has nothing to do with offshore. If that is indeed the case, does MMS or some other agency provide guidance on change or is it all up to the drilling / production company?

There is a Management of Change process that does exist both for BP and Transocean. It is extensive and requires documented reasons for changes as well as after the changes are made full documentation of what was done.
If there was over site it would have to either be by MMS or the Coast Guard.

Ok. Thanks Tigerfred. It will be interesting to see if MOC process was used on these modifications to the BOP!
