The first thing is there has to be room in the server. Then there has to be room on our team. Then there has to be room in our squad.
We should always create our own squad so that we have three spots open. It's up to us to invite them or them to join us. That's really the best way to do it right now.

No, once you get in. Hit Options on the controller (old Start button), then whatever it is to leave squad (X, I think), then whatever button to create a squad (triangle, I think).
