How can the ball not be possessed during the play if 54 came away with the ball? Did the ref blow the whistle before possession?

You'll see it when it's a mad dash in the open field and if someone dives on the ball clearly and is downed, but has ripped out after a pile forms. The person who comes out of the pile is not for sure the actual person that recovered the ball

The NFL has repeatedly established that a player coming out of a pile with the ball after its been blown dead does not constitute a clear recovery. It happens 3-4 times a year and it has NEVER been ruled a clear defensive recovery.
