You are actually not too far from having a very technically sound swing...
1. Slow your swing down. You are WAY too fast. Drill: Exaggerate the slowness at a 60% swing speed for 10 swings then hit a normal shot.
2. Your shaft angle at impact is not forward or perpendicular, but "back slashed". Focus on keeping your shoulders square to the ball longer. You are losing shaft angle by releasing the angle way to early. Drill: bring a wedge back to about half-way, and swing through the ball with your right shoulder, maintaining as much of a wrist angle as possible throughout the small swing.

Nothing wrong with that. I'll try to get my swing posted on here soon(I hit balls at Santa Maria if anyone ever wants to meet up).
