How is it the same?
Are there significant numbers of people who want their arms or legs lopped off? I can't relate to either random proposition really but assume the psychological/physiological motivations are different

not in the way fat people are,,but they feel like a part is missing or theres a part there that should not be
and yes its rare,, but 50 years ago so was sex reassignment surgery

You don't think there's a difference?
I would assume the difference between wishing your body was a little more attractive (by our social standards) and wishing you were a different sex is a pretty large one... almost everyone wishes their body were a little different... very few people wish they were a different gender IMO
FWIW, I'm not sure either side of the argument is necessarily wrong. If you believe things like gender reassignment are ethically equivalent to liposuction I guess my bias is to disagree but maybe more people in the future will agree with you (essentially that it is no big deal?)

I wonder how many people felt this way years ago when the surgery wasn't possible. Were there guys who said "Gee, I wish a doctor could split my dick in half, tear off my balls and fashion a makeshift vagina. That'd be super," or did some quack one day announce that this was possible and battered tranny's flocked to him?

I think society as a whole is far more indulgent, and once the first few had it done, others were more willing to do it. Getting your arse kicked because the guy you brought home discovers you have a wang is very off-putting, I suppose.
Either way, I suspect hard drugs were involved in the process.

I'm not sure if I agree with that. Cosmetic procedures are becoming more and more common in just the last 10-15 years (these numbers also don't seem to include Americans who travel out of the country for these procedures for reasons of cost or privacy).
