see a vet. dog likely needs a dental and workup up on potential periodontal disease.

Ours recently had a mouth that smelled especially like butthole. Took him for his teeth cleaning and they found one of his back teeth was moving towards being rotten at the root, so they took it out. Fixed the problem almost immediately. I'm not saying that's your dog's problem, but oral health is a huge indicator. Have the vet check for any inflammation or rotting.

Probably his anal gland. Makes breath really bad when it needs to be expressed.

greenies tend to help a little. Biggest thing you can do is actually brush your dogs teeth. They make doggie toothpaste. If your dog is that old and has bad teeth do be gentle and careful. how old is your dog and breed? you could always ask for pre-anesthetic BW to see his values. if everything checks out would be less of a worry.
