If he is asymptomatic, no high bp or diabetes, tell him to lay off protein for a week and get his creatinine re-run.
Creatinine results drive the eGFR calculation that leads to the designated stages of CKD.
It’ll be high if he’s doing keto or low carb diets.
Edited to add: non-professional opinion, just what I did. eGFR goes down as age increases too. It’s just a calculation, so symptoms matter more than eGFR

I was diagnosed with stage 3 about 12 years ago. I made a lot of changes to my diet and increased water etc., but it continued to decline.
Eventually they did a biopsy and determined that I have IGA Nephropathy. Excess immunoglobulin A which wrecks the vessels in the kidney.
Had to incorporate a low potassium diet into my regimen once I hit stage 4, so I stopped eating avocados, bananas, potatoes, milk, and limited my cheese and fruit intake.
My nephrologist was able to refer me for transplant before I had to start dialysis. My function was at around 18% when I was referred.
My whole family got tested to be a donor, and my sister was a 4/4 match. I was referred in Jan 2022. By the time I got the transplant I was at around 13% function but was able to avoid dialysis.
I got the transplant on Aug 10, 2022. So I’m 6 weeks post-op. The immunosuppressants I’m on should help keep the IGA in check. My nephrologist said there’s no reason I can’t get another 30 years on this new kidney.
I bounced around with several nephrologists, and even went to Mayo Clinic in Tallahassee for a second opinion. I finally found a really good nephrologist in New Orleans that was referred by the Mayo Doctor: Joshua Lowentritt.
I got the transplant at Tulane Medical in New Orleans, and they were great. I had considered going to Mayo for the transplant, but there’s so many appointments before and after that it didn’t make sense for me and my donor sister to travel so much.
