If it’s in your urethra, it’s likely not causing the intense back pain because it’s already made it’s way through your bladder and is on the way out.

The pain is when the stone is stuck in the ureter and is pushed through toward the bladder by urine from the kidney. And the backup pressure and spasms are the painful components.
