When using to cut with a knife I'll go upside down with the left hand to hold what I'm cutting then twirl it between the last knuckles on my thumb, middle finger, and pointer to put in my mouth. Any other time it's regular with the right.

I just hold the plate up to my mouth and use the fork like a rake.
