Maybe little a reality check... this OP's 'wife' who is approaching 40... why do we assume that she's even attractive?
IMO: here is a chart to gauge the amount of worry the OP should be:
If the wife's body type = the two on the left -> legit worry
If the wife's body type = the third -> eh... little concern
If the wife's body type = the fourth -> if banging this 'loser' college friend keeps her happy... and she continues to cook and clean the house...
