I have no idea what it could be. Almost looks like something is sticking out of it but could be a rock. Trail cam pics can be tricky sometimes.

Cancer Eye. Not totally uncommon in cattle, especially in some breeds. I have had several that had it over the years, there is no cure and if you try to sell it the carcass is condemned.
