My brother downloaded that awhile back. His credit card info and PayPal account was hacked soon after. Noticed the credit card charges the next month. Could be coincidence.

I have been sending an email every day for like 3 days and I haven't got activated yet. I don't think it works anymore

To use Apple TV, I swipe the bottom, click AirPlay and also switch on mirroring

If you haven't already downloaded it then you can't get it from the app store anymore. I haven't checked but I read that it's available from Cydia if you're jailbroken. If you do have it you can backup the ipa to a computer and then not worry about losing it if your iPhone or iPad craps out suddenly.

I've been trying to get activated for 2 weeks and I still haven't been activated.
I go to settings then contact us and an email box pops up. I type requesting activation in the body of the email while leaving the codes but I still have gotten no response.
Is there something I'm doing wrong?

Are you sure you haven't been activated? I never got the pop up but now I have a little play button under the movie when I click one.
