I am not sure about present day, but in the past their 'home" ice was Planet Ice in Lafayette. That is still the address listed on their "contact us' page.

For the posters posting about the waiver being denied need to read both parts as one whole statement. It says “it would be absurd to conclude that the info provided does not constitute adequate medical documentation”.
