I Need a Ruling on Negligent Roster Management

I need an arbitrary ruling on a meltdown going on in a league.
Competitive league. Everyone cares. One guy was God awful last year and in week 12, forgot to replace a bye week receiver. Anyone on his bench would have given him the win, but he lost thus giving his opponent the win for the week which ended up vaulting the opponent into the playoffs over another team.
We talk about punishment for after the year but nothing gets finalized. It happens again this week with the SAME TWO TEAMS. He swears he checked his roster, but ended up leaving Willie Snead in which was the difference in the matchup. This potentially affects the entire league because all but two teams are within 2 games of one another.
I'm going ape shite because I originally vouched for harsh penalties (loss of 2nd round draft pick, keeper league) so this wouldn't happen again and got push back for being to severe. We aren't doing anything about it this year because the consequences weren't made official.
What would you guys set as punishment? Sorry if TLDR.

If it’s happened twice to the same person, then boot him.. that’s BS and ruins the ‘integrity’ of the league... call a vote, if it doesn’t pass, then quit the league to send a message. I’m sure you aren’t the only one who is pissed, but your actions might give them a little nudge to also speak up

Kick him out.
Had the same thing happen in my league last year. Talk of banishment occurred. In the end we let him stay for this year, but said he better not give up and not set his lineup at any point. So far, so good. But if he does it at some point this year there will be permanent recourse taken.

He's close friends with the commissioner and several of the members. He won't get booted. You would think there would be more pushback than there is, but most people are taking the "shite happens" approach. Even the guy who it happened to last year is saying, "well, he did check at one point and leaving a hurt guy in isn't as bad as leaving a bye week player in.", even though he has a similar record and it could very well happen to him again!

Not really going ape shite. Just the most vocal against what happened.
It didn't affect me last year and I was vocal. It has the chance to this year and I'm just as vocal. It shouldn't matter if it directly affects me or not. It's the integrity you speak of.

I agree with you 100%, I’d be pissed too. I was just offering a reason as to why nobody else was as mad as you

That stinks but the bye week players are much worse. Snead was a surprise inactive so I might cut him slack there as opposed to playing a guy that was obviously supposed to be out.

The thing that pisses me off about these types of situations is that in my case, the guy who ‘forgot’ to set his lineup has his damn phone in his hands 24/7... come on man, you just said ‘frick everyone’ cause you started 1-3

Agree with the above. Snead was inactive and that news came out about an hour and a half before kickoff.

When was the snead announcement made? If it was a relatively last minute announcement I might be more inclined to give the guy some slack. You could over sleep and get screwed by something like that.

I set an alarm for 11:30am to check questionables for this exact reason. It can easily be avoided.

No he wasn’t doubtful on Friday. He was questionable which this year means expected to play.
“Willie Snead (hamstring) is questionable for Week 7 against the Packers.
He should be ready to roll after getting in a limited week of practice. Snead played just 27.3 percent of the Saints' snaps in his season debut last week, but should play a much bigger role against the Packers. Oct 20 - 4:15 PM
