Because they sell and they are profitable.
I think the Ford Econoline, GMC Savana, and Chevrolet Express are designs that date back to the 1990s. The tooling has probably paid for itself 10x over. And since the designs are so static, they also are very likely to reach 500,000 miles even with a lot of abuse. People tend to replace these things with the same make/model every time they need a new vehicle in their fleet. So why change?
They can also do a cutaway for ambulances and box trucks very cheaply with the designs that they have, and I think the Express Van cab is what Chevrolet uses for their medium duty truck cabs so they aren't in a rush to change the design very often.
