If I do fast food..At McDonald's for breakfast the sausage biscuit and a big coke..I avoid the McGriddle because it gives me real big gas..Found that out on a road trip. Passengers were not happy.
Other times I get two of the small burgers and a big coke..

Dang, 5 pages and I guess I'm the only one who likes their fish sandwich and an order of hot fries. About every 3-4 months gotta have one.

The one near me is really, really good about having a 24 hr drive thru that is never open past midnight.

I loved the Filet-o-Fish as a kid. A Friday afternoon Lent tradition. Haven't had one in years though.

I like their regular hamburgers. Ketchup, mustard, pickle, onions. Simple but good.

Depends on the location. Some make them from scratch and some get them already made. It's one of the few times you'll find something different from store to store (in the same market).
It doesn't drive the cost up much (a couple of pennies as of a few years ago), but you do need a dedicated person taking care of it. In a restaurant that is already by far one of the most complicated to run this is an extra step many owners don't want to deal with.
