everything that happened on the island, really happened, everyone who died on the island, really died, including Jack.
Sawyer, Kate, Claire, Frank, Richard and Miles got off of the island on the plane and lived out the rest of their lives until they eventually died.. like Christian said "Everyone dies sometime, kiddo. Some of them before you, some... long after you."

They died when they died on the show. As for the ones who got off the Island, they lived long lives. Rewatch the scene with Christian and Jack, because it carefully explains what happened.

I'm not trying to a butthead, but it amazes me that some people can watch all of LOST, including the finale, and have questions if it they were alive or in purgatory or whatever. Christian point blank, black and white says it. Some died before you, some long after...
