Penthouse suite scene in Oldboy
Ending of Arlington Road. Still one of the most screwed up endings to a movie I've seen. Real depressing.

The end of the season finale of Dexter season 4. Never been that stunned before from a tv show.
I stood silent on my couch for at least 20 minutes after the show had ended, my mouth just gaped open. I agree with you. I've never been that shocked from a television show in my life.

I'd like to add the zombie baby birth scene in the Dawn of the Dead remake. Fricked up.

"Why would you help her?" scene in The Ring. You think the movie is over, but... wow.
The burlap sack in Audition.
John Travolta's death in Pulp Fiction.
Blanking on a lot of others, but those got me.

Horse head in the bed. I knew about it before but it was still something else.

Hands down the most SHOCKING scene for me, as far as content in a particular film:
"Doc Hollywood" starring Michael J. Fox in 1991.
It was a PG-13 movie.
Full-frontal female nudity (Julie Warner) about 30 minutes in. A prolonged shot, even.
Again... PG-13. How the frick did THAT make it in?

I'd watch that movie just for that scene.

i always thought the cat scene was an homage to the marvin scene. at least that's how i took it even if it wasn't intended that way.
when i saw silence of the lambs in the theatres, hannibal's escape scared the hell outta me. and i knew it was coming.
i agree with those who said the ending of departed.
"what's in the box" although the build up to the reveal takes some of the shock out of it.
curb stomp in american hist X
carrie- hand popping out of the ground
a bunch of shite in the exorcist

Exorcist III
Almost shat my pants first time in that scene.
Leo in The Departed or Omar Little.

Lots of good ones already mentioned. I'll add:
The last scene of Deliverance, close up half submerged camera shot of a serene lake at dawn, then a hand shoots out of the water! Cut to Jon Voit waking from a nightmare in a cold sweat!
