My main reason isn't listed .. although I have many of the reasons you do list.
Out of EVERY employee, contract or company person, I AM THE ONLY ONE BEING MANDATED TO HAVE THE SHOT before I can go in the building. And NO I'm not shitting you ... I'm contract and being mandated to take it -- no nurse, no CNA, no therapy person -- NOT ONE is being mandated besides me.
NO ONE can give me any good reason why it's this way. Oh, and if I want to "visit a resident just because", I CAN do that unvaccinated BUT I can't assess them on admit unvaccinated.
DUMB RULES are why I'm not .. along with no comorbidities, not super worried, Vit D levels and wt are good, AND my HCP has told me that she sees no immediate reason for me to rush to take it.

I’m not anti-vax,I just want to wait and see if any long term side effects show up.I had the flu about 25 years ago and felt like I was going to die,worst part it took me couple of months to get back to normal.I’ve taken flu shots religiously since and haven’t gotten flu since nor have I had any side effects.
I’m just unsure about this vaccine considering how rushed it was and that it’s still under emergency use authorization.
I don’ give 2 shites that the majority of Drs.are pushing it.Drs. just engage in group think and push what sounds good.In my 35 year nursing career it was amazing the number of established medical practices were later found to be useless and in some cases detrimental.

I broke down and got my first Moderna vaccine yesterday .I basically had to since I’m an ICU RN ! I feel like I’ve been hit by a truck. Joint aches , chills, sweats , don’t like it at all .

Thank you, not mandatory but you get a sense that it’s only a matter of time . They put a guilt trip on you. I could be a carrier and give to my pt’s or family.
