Phaethon's Brethren - "I Put A Spell On You" (1965-6?)
Well here's an intriguing little mystery: a magnificent cover of the Screamin' Jay Hawkins R&B classic, with some great organ and nice use of horns. Despite its brilliance this track was never released, and survives only as an acetate. Little else is known about the band.

I'm really not a fan of Disturbed's version of the song. The dude can sing, don't get me wrong, but it was just too... bombastic for my taste.

fwiw..imo The Grateful Dead are probably the greatest live cover band ever. They have too many fantastic live version covers to list.

For me there is no contest - it is this unbelievable a cappella rendition of Billie Holiday's "I'm A Fool To Want You" by a seven year old Angelina Jordan, with added backing track that illustrates her absolutely perfect pitch without the need to even hear the music, and in a minor key. Yes, that IS her real voice.
EDIT: Oh... and in a FOURTH language - English. She primarily spoke Norwegian (birthplace, school), Farsi (Mom, Grandma), Swedish (Dad), and some Japanese (Grandpa).

I don't think either of these are remotely in consideration for the best cover ever, but I like Joan Baez's The Night They Drove Old Dixie Down as well as Linda Rondstadt's cover of Willin' - Little Feat.

If you're talking live shows, I saw Drive By Truckers close a show at the Lyric in Oxford with "Keep On Rockin in the Free World" that was really awesome.
At Memphis in May Music Fest about 20 years ago Hootie and the Blowfish closed with 3 Led Zepplin songs, I don't remember all 3 but I remember they did a great job with "Over the Hills and Far Away"
