Like others have said it's a camera case. I believe that photo is from the set of The Bridges of Madison County in which Clint played a National Geographic photographer.

I had to hold Mrs. S’s diamond bordered fendi bag as our driver was occupied speaking with another valet. I was a bit uncomfortable for a split second but honestly it’s not much different from holding my louis vuitton toiletries bag, aside from the tucked arm positioning.
