We have some 60+ yr old pressure vessels, ~6ksi, that weren’t even built to code initially. Some layered, some T-1…all questionable. Fracture mechanics to develop remaining safe-life on those is challenging. Hell, we have Div 2 vessels from the 90’s that show all sorts of LOF when subjected to PAUT examinations.

It was a 16” line so it’s DOT regulated. Smart pig ran at a minimum once every 5 years.
I’m sure they saw corrosion, there is tons of corrosion all over every pipeline. You repair first what is required by regs, then repair what internal company guidelines you put in place. It sounds like the corrosion reported wasn’t a fed required repair.
The issue is every smart pig company is different and the tools usually have at least a +/- 10% threshold for what they call. You have to do analysis based on the depth and length of the anomalies on what causes pressure issues or not depending on what the pipes operating pressure is at.
What likely happened was the smart pig under-called the Anomaly and they either didn’t select this area for field investigation or they haven’t completed that work yet.

I don't understand how it went from "not that bad" to "rupture" in 14 months.
TO me this is like the Florida condo. You see the obvious corrosion and decide, meh, it's fine, not gonna fix it until someone makes me! Government is corrupt and incompetent so no one makes them, or apparently even makes them keep an eye on it.
Also do these pipelines not just auto shut down / stop flow when there is a loss of pressure, which I assume with a leak, there would be?
