I hyper-extended mine about three years ago and it does hurt like a mofo. I still suffer with pain.... However the clicking and crunching it makes when I bend it is awesome and freaks the wife out all the time.

The severity of the immediate pain is not perfectly correlated with the severity of an injury.

My best friend tore his ACL in high school and didn't realize the severity until it became unnaturally swollen later that night.

On the other hand, cramps can make walking nearly impossible.

I still remember the pain from tripping and landing my kneecap directly into the edge of a metal table base. That's the worst pain I can ever remember feeling. I broke my arm when I was a like six(so I don't overly remember the pain), but I do remember crying on the side of our house holding my broken arm up, more willing to endure that than the anger my mom would unleash on me for jumping on our neighbors trampoline alone.

I can only assume that means the knee(at least initially) hurt more than my broken arm did.
